New ICWP Clients
For individuals entering the Independent Care Waiver Program and seeking case management services.
Waiver-to-Waiver Transfers
For eligible individuals transitioning from another waiver program.
Case Manager Transfers
For individuals already receiving services who are interested in transferring their case management services to Life Goes On.
