Additional Coordination for Complex Care Needs
Enhanced Case Management provides a higher level of care coordination for eligible individuals enrolled in Georgia’s Independent Care Waiver Program (ICWP).
This service is designed for individuals with complex medical, physical, behavioral, or support needs who may require more frequent monitoring and coordination to live safely in their homes and communities.
What Does an Enhanced Case Manager Do?
An enhanced case manager works closely with the individual, family members, caregivers, healthcare professionals, and service providers to ensure that approved services are properly coordinated.
Enhanced Case Management may include:
Completing a comprehensive assessment of the individual’s needs
Developing a person-centered plan of care
Coordinating authorized ICWP services
Maintaining increased contact based on the individual’s needs
Monitoring health, safety, and changes in condition
Communicating with physicians and healthcare professionals
Coordinating with caregivers and service providers
Addressing concerns involving service delivery
Supporting transitions from hospitals or other facilities
Connecting individuals with community resources
Updating the care plan when needs change
Developing backup plans for essential services
Advocating for the individual’s choices and goals
Who May Need Enhanced Case Management?
Enhanced Case Management may be appropriate for eligible ICWP participants who:
Have significant physical disabilities or traumatic brain injuries
Have complex medical or behavioral needs
Require a hospital level of care
Need increased monitoring and service coordination
Receive support from multiple providers or caregivers
Are transitioning from a hospital or facility into the community
Need additional assistance maintaining health and safety at home
The appropriate level of case management is determined through assessment and authorization under Georgia Medicaid requirements.
