Our Case Managers work with eligible ICWP participants to coordinate services and address needs while supporting the individual's ability to live as independently as possible in their home and community.

What to Expect
01 — Referral
Contact Life Goes On or submit a referral to begin the process.

02 — Review & Onboarding
Our team reviews the referral and determines the appropriate next steps.
↓
03 — Initial Meeting
Once accepted, an initial meeting is scheduled to discuss needs, services, providers, and the client's Care Path.
↓
04 — Meet Your Case Manager
The Case Manager Supervisor introduces the client to their assigned Case Manager.
↓
05 — Ongoing Case Management
The Case Manager continues working with the client to address needs, review goals, and provide ongoing case management support.
