
Traditional Case Management helps individuals enrolled in Georgia’s Independent Care Waiver Program (ICWP) coordinate the services and support they need to live safely and independently in their homes and communities.
A traditional case manager works closely with the individual, their family, caregivers, healthcare professionals, and service providers. The case manager helps develop a person-centered plan of care, coordinates approved services, monitors the individual’s progress, and responds when their needs change.
How We Help
Traditional Case Management may include:
Assessing the individual’s needs, strengths, and goals
Developing and maintaining a person-centered care plan
Coordinating authorized ICWP services
Connecting individuals with service providers and community resources
Monitoring the delivery and quality of services
Communicating with families, caregivers, and healthcare professionals
Addressing concerns or changes in the individual’s condition
Advocating for the individual’s choices, safety, and independence
Completing ongoing reviews and required documentation
Who Is This Service For?
Traditional Case Management is intended for eligible ICWP participants. ICWP serves qualifying adults with significant physical disabilities or traumatic brain injuries who need support to live in a home or community setting instead of a hospital or nursing facility.
Eligibility and services must be approved through Georgia’s Medicaid program.
