Supporting Recovery, Independence, and Community Living
A traumatic brain injury can change how a person thinks, communicates, moves, behaves, and completes everyday activities. These changes may create long-term challenges for the individual and their family.
Life Goes On Case Management Services helps eligible individuals with traumatic brain injuries coordinate the services and support they need to live safely and independently in their homes and communities.
What Is a Traumatic Brain Injury?
A traumatic brain injury occurs when an external force causes damage to the brain. It may result from a fall, vehicle accident, sports injury, violence, or another serious impact to the head.
Every brain injury is different. Its effects may be temporary or long-term and can range from mild to severe.
How Can a TBI Affect Daily Life?
Depending on the location and severity of the injury, a person may experience changes involving:
-
Memory and concentration
-
Speech and communication
-
Mobility, balance, or coordination
-
Decision-making and problem-solving
-
Mood, emotions, or behavior
-
Personal care and daily activities
-
Vision, hearing, or sensory processing
-
Fatigue and energy levels
-
Social relationships
-
Employment or education
-
The ability to live independently
Because needs can change over time, ongoing care coordination may be an important part of supporting the individual’s safety and quality of life.
How Case Management Helps
Our case managers work with the individual, family members, caregivers, healthcare professionals, and service providers to develop a person-centered plan.

Coordinating authorized home and community-based services

Connecting the individual with qualified providers

Monitoring health, safety, and service delivery
-
Communicating with caregivers and healthcare professionals
-
Supporting transitions from hospitals or rehabilitation facilities
-
Identifying community programs and resources
-
Addressing changes in the individual’s condition
-
Developing emergency and backup plans
-
Advocating for the individual’s choices and preferences
-
Encouraging independence and community participation
Support Through Georgia’s ICWP
Georgia’s Independent Care Waiver Program helps eligible adults with significant physical disabilities or traumatic brain injuries receive support in their homes or communities instead of hospitals or nursing facilities.
Depending on an individual’s assessment and approved plan of care, ICWP services may include case management, personal support, skilled nursing, specialized medical equipment, environmental modifications, and other authorized support.
Who May Qualify?
ICWP generally serves eligible Georgia residents who:
-
Have a significant physical disability or traumatic brain injury
-
Apply between the ages of 21 and 64
-
Meet Medicaid financial requirements
-
Require a nursing-facility or hospital level of care
-
Can live safely in a home or community setting with appropriate support
-
Meet all applicable program and clinical requirements
Final eligibility and service authorization are determined by the appropriate Georgia Medicaid representatives.
